The aim of the study was to compare lipid and lipoprotein behaviour after an oral fat load (1 g fat/kg body weight) in patients affected with multiple symmetric lipomatosis (MSL) and in healthy controls of the same age range. Metabolic features of MSL include elevated adipose tissue lipoprotein lipase activity and hyperalphalipoproteinaemia, so intestinal lipoprotein catabolism was supposed to be very efficient in this condition. Triglyceridaemic response after fat meal was significantly reduced in patients compared to controls and showed a significant negative correlation with high density lipoproteins (HDL), HDL2, HDL3 and apolipoprotein AI in the pool of subjects studied. These results demonstrate that HDL levels are a major determinant of post-prandial lipoprotein metabolism. Therefore, the lack of atherosclerotic disease observed in MSL may, at least in part, be justified by a very high efficiency of removal of remnant lipoproteins
