Proposta per una cartella-libretto personale pediatrica

The Author's purpose is to introduce a new medical-file for continuous and standardized health supervision of children from birth to 14-years of age. The model has been devised in order to meet the recording needs of the community's health services, furnish information to families and act as a data source for epidemiological surveillance. It is laid out in such a way to give a medical file and, at the same time, personal memorandum and precoded sheets for computer analysis. The operational scheme will forsee free and scheduled medical examinations for each single child. The timing of fixed assessment procedures has been chosen in order to select accross anamnestic and physical data, certain groups who are at risk, and who will be treated according to specific protocols. In this work the Authors focus the follow-up program for monitoring growth, psycho-motor, visual, auditive and speech development as well as dietary staus, bone deformities, teeth eruption and immunization schedules.
