Treating soft tissue sarcoma of the extremities requires a multidisciplinary team in a referral sarcoma center. Combining chemotherapy, radiotherapy, and surgery is crucial in high-grade, deep, and large soft tissue tumors. Limb salvage surgery with wide margins is the gold standard of surgical treatment. Indeed, although it is not wholly demonstrated, surgery with wide margins seems related to low local recurrence rates and high disease survival. Resection of nerves or vessels involved in the tumor is mandatory; however, resection of the bone in contact with the tumor is debated. If wide margins are not achievable, amputation could be necessary.
