30/48 (62.5%) patients examined surgically for radiographical pictures showing a roundish opacity in the lung had a primary or metastatic tumor. The remainder displayed benign lesions. The limitations of radiography, bronchoscopy and other instrumental examinations in the formulation of preoperative diagnosis are discussed. Radiography was able to get beyond description of the lesion only in a few cases. Conventional bronchoscopy gave a positive result in cases with a hilar site. The risks associated with transcutaneous needle biopsy are such that it can only be used when involvement of the chest wall is certain. Stress is thus laid on the advisability of subjecting such patients to thoracotomy on account of the low risk associated with this operation.
