One third of primary parotid tumors are malignant. Mixed tumors and Warthin's tumor are the more common benign neoplasms. Adenocarcinoma and mucoepidermoid carcinoma are the frequently occurring malignant tumors. Surgical treatment must include wide resection or total ablation of the gland avoiding damage to the facial nerve. Block dissection of draining lymph nodes is indicated only when there is evidence of lymph node metastasis or in highly undifferentiated tumors. Facial nerve paralysis and recurrence of the neoplastic growth are the more frequent complications.
