A thymolipoma of considerable volume (1200 g) was removed surgically from a man aged 44 yr. The difficulty of differential diagnosis, the rarity of this morbid form, the exact nature of which can be identified only by histological examination, the precautions to be taken before the operation and the ease of surgical treatment are described.
