To the Editor: In the July 19 issue of the Journal, Davis and his colleagues1 reported that captopril may have beneficial effects when congestive heart failure has been poorly controlled by conventional therapy. These findings suggest a possible role of the renin-angiotensin system in congestive heart failure and emphasize the potential therapeutic value of pharmacologic manipulation of the renin-angiotensin axis in this disorder. In this context, digitalis glycosides deserve particular mention because they may interfere with renin secretion through different mechanisms, such as antiadrenergic activity,2 inhibition of ion transport in the juxtaglomerular apparatus3 and effects on hemodynamics.
