Evidence for biochemical, functional and histologic features of hyperparathyroidism was investigated in 20 patients with bilateral nephrolithiasis 10 of them with renal insufficiency. Tm(po 4) values related to functioning nephron mass were reduced in patients with renal insufficiency ('obstructive nephropathy'): at comparible levels of renal insufficiency, the reduction of Tm(po 4) values was more evident than that observed in a group of patients with chronic glomerulonephritis, taken as controls. The histologic appearance of bone tissue was characterized by severe osteomalacia and osteoclastic resorption in most patients with obstructive nephropathy. This preliminary investigation suggests that secondary hyperparathyroidism is early, frequent and severe in obstructive nephropathy more than in other chronic renal disease of glomerular or vascular etiology.
