Harasymowicz et al (1) are to be congratulated for their recent article regarding the presence of regional differences between perisynovial and infrapatellar adipose tissue depots and their response to Class II and III Obesity in Patients with osteoarthritis (OA). The Authors report that the infrapatellar fat pad (IFP) and synovium adipose tissue depots differ significantly and are influenced by the patient's BMI. In particular, obese patients' adipocytes from IPF are significantly larger while in the synovium, there was marked fibrosis, macrophage infiltration and higher TLR4 expression identified in comparison to lean subjects.
