This report describes an unusual case of fungal peritonitis, due to Fusarium verticillioides in a patient on chronic peritoneal dialysis. To our knowledge this is the first time that species verticillioides of genus Fusarium has been identified as the causative agent of peritonitis. In contrast to previous reports of fungal peritonitis, the early phase of the illness was characterized by a clear effluent. Owing to the absence of vascular access, the Tenckhoff catheter was not immediately replaced and only a medical treatment was instituted. Twenty days later the peritonitis appeared resolved; but, after a month, the fungus reappeared and the patient died of cardiovascular arrest. We suggest that Fusarium peritonitis may have a clinical picture peculiar to itself, at least in its early phase.
