The update of the Table 3 of the Italian Official Pharmacopoeia XII ed. represents an important step to the harmonization of its content to the international current legislation. However, this article points out some problems occurring in the professional activity both of the Physician and of the Pharmacist within the galenic field. Moreover, the Authors suggest a further intervention by the legislator to overcome some discrepancies and obsolete rules, with special regards to the safety of the galenic medicines, the transparent information to the medicine user, the clinical risk management within galenic field.

Abstract
The update of the Table 3 of the Italian Official Pharmacopoeia XII ed. represents an important step to the harmonization of its content to the international current legislation.
