Pancreas could be the metastatic site of recurrence of a previously resected clear cell renal carcinoma many years later. We report the case of a single metastasis at the pancreatic tail 33 years after the index nephrectomy. Because of the positive outcome after surgery, radical resection of both solitary and multiple metastases should be attempted in all surgically fit patients.
